Medical weight loss options

Treatment options

Couple running outdoors during a medically supervised weight loss plan
  • GLP-1 medications

The core of most plans. They act on the hormones that control appetite and blood sugar, and many patients feel full sooner and notice fewer cravings. Our GLP-1 weight loss medications page compares the options and covers side effects.

  • Semaglutide

The active ingredient in Wegovy and Ozempic. In a 68-week clinical trial of FDA-approved semaglutide 2.4 mg plus lifestyle changes, adults lost an average of about 15 percent of their starting weight (PubMed). See our semaglutide page.

  • Tirzepatide

The active ingredient in Zepbound and Mounjaro. In a 72-week clinical trial, adults on the highest dose studied lost an average of about 21 percent of their starting weight (PubMed). See our tirzepatide page.

  • Hormone support

When perimenopause, menopause, or low testosterone is part of why the weight will not move, hormone optimization can be part of the plan. Women dealing with midlife weight gain can read more on our menopause weight gain page.

  • MIC-B12 injections

An optional injection containing methionine, inositol, choline, and vitamin B12 that may be offered alongside your main weight loss plan when appropriate.

  • B-Lean IV

An optional nutrient infusion that may be added to your plan after evaluation.

Brand-name and compounded options

FDA-approved medications for chronic weight management include Wegovy (semaglutide) and Zepbound (tirzepatide). Ozempic also contains semaglutide, and Mounjaro contains tirzepatide, but those brands are FDA approved for type 2 diabetes rather than specifically for weight management. Eddie prescribes based on your health history and treatment plan.

When an FDA-approved medication cannot meet a patient's individual medical needs, a provider may determine that a compounded medication is appropriate under federal and state requirements. Compounded medications are not FDA approved, and FDA does not review them for safety, effectiveness, or quality before they are marketed. Compounded versions were not used in the clinical trials discussed above.

Regional health data

Weight is one of the region's most common health problems

Across the Tri-Cities, roughly four in ten adults have obesity. CDC PLACES modeled estimates for 2023 put adult obesity between about 35 and 41 percent across several counties and communities in the region.

Source: CDC PLACES county data, 2023 modeled estimates of adult obesity. Bars are drawn on a 0 to 50 percent scale.
County or cityAdults with obesity (modeled estimate)
Washington County, TN35.5%
Sullivan County, TN37.8%
Carter County, TN37.9%
Bristol, VA39.2%
Unicoi County, TN39.3%
Greene County, TN40.6%

Those numbers are your neighbors, your coworkers, and the people in line with you at Food City. If you have been fighting your weight for years, you are in a very large group.

And most of that group has heard the same advice: eat less, move more, try harder. It rarely holds, because the body fights back when you cut calories.

Calorie restriction can increase hunger and reduce the energy your body burns, which makes weight loss harder to maintain. Medical weight loss treats that as the health condition it is.

Why the weight will not move

What is blocking your weight loss?

For a lot of people it is not one thing. It is some mix of:

  • Appetite and hunger signals that never switch off
  • Blood sugar that swings through the day
  • A thyroid that is running slow
  • Hormone changes in your 40s and 50s
  • Medications that make weight harder to lose
  • Years of dieting that cost you muscle and slowed your metabolism

That is why your first visit starts with labs and a body composition scan instead of a meal plan. A good plan starts with knowing what you are working with.

Is this for you?

Who medical weight loss is for

If you want to lose weight, you can start here.

Every plan begins with real numbers. You get a body composition scan and lab work, so you know what is actually going on before anyone talks about medication.

Not everyone needs a GLP-1. Your labs and your history decide what fits, whether that is medication, hormone support, nutrition changes, or a mix.

When a GLP-1 is on the table

Wegovy and Zepbound are FDA approved for adults with a BMI of 30 or higher, or 27 or higher with a weight-related condition like high blood pressure, type 2 diabetes, or sleep apnea.

Eddie takes a closer look if

  • You or a family member has had medullary thyroid cancer, or you have MEN 2. GLP-1 medications are not used in these cases.
  • You are pregnant, breastfeeding, or planning a pregnancy soon.
  • You have had pancreatitis.

He reviews your full health history and current medications before prescribing anything. If medication is not the right call for you, he will tell you plainly and help you find what is.

Your first steps

How medical weight loss works at Peak Performance

  1. Consultation

Eddie goes through your weight history, current medications, health conditions, and what you want out of this. Plan on 45 to 60 minutes.

  2. Labs and a baseline scan

Bloodwork that can include thyroid and blood sugar markers, plus a body composition scan that separates fat, muscle, and water.

  3. Your plan and your price

Medication if it fits, nutrition and strength targets, and the monthly cost, all before you commit.

  4. Weekly injections at home

We teach you how, so your dose does not require a drive.

  5. Follow-ups and adjustments

Dose changes, side effect management, and repeat scans as your body changes.

  6. Maintenance

Stopping takes a plan too. Research shows most people regain a large share of their weight within a year of stopping without one. When you reach your goal, you and Eddie decide whether to taper, hold a maintenance dose, or come off, and we keep checking in while your new habits take over.

We are in this with you

This takes work. You will not be doing it alone.

We are going to be honest with you. The medication quiets the hunger, but it does not build the habits for you.

The patients who do best treat the first few months as training, not a shortcut. The work is doable, and you do not do it by yourself.

Your part

  • Give yourself your weekly injection
  • Eat enough protein, even when you are not hungry
  • Strength train 2 to 3 times a week and move most days
  • Come to your follow-ups
  • Tell us when something feels off: side effects, a stall, a hard week

Our part

  • The same provider every visit. Eddie knows your history and adjusts your plan as you go.
  • Call or message us between visits. Questions, side effects, refills. You do not have to wait for your next appointment.
  • Regular check-ins, where your dose, nutrition, and plan change as your body does.
  • A body composition scan at your visits, so you can see fat coming off and muscle staying on, even when the scale is slow.
  • Protein and meal guidance built around eating less overall.
  • A plan for side effects. If nausea shows up, we have specific ways to handle it and can slow your dose increases.
  • Someone in your corner when you stall. You work through a plateau with a provider instead of quietly giving up.
  • A maintenance plan, so reaching your goal is not the end of your care.

We are not here to write a prescription and disappear. We are here until the weight is off and staying off.

Your first weeks

What your first month feels like

Most people notice the food noise get quieter within the first few weeks. Meals get smaller without a fight.

The first month is also when your body adjusts, so it helps to know what is normal and what to do about it.

What helps in the first few weeks

  • Eat slower and stop before you feel full. Your stomach empties more slowly now, and fullness shows up late.
  • Keep meals small and simple. Greasy, heavy meals are the most common trigger for nausea.
  • Eat protein first. It protects your muscle when you are eating less overall.
  • Drink more water than you think you need. Dehydration makes fatigue and constipation worse.
  • Call or message us early. If something feels off, we can adjust your plan before it turns into a bad week.

At your follow-ups, Eddie looks at how you are feeling, your weight, and your body composition, then decides whether it is time to move your dose up or hold where you are.

Before you choose

Online GLP-1 program or an in-person clinic?

You have probably seen the ads for GLP-1 prescriptions by app. For some people that works. Before you choose, ask any program these questions.

Ask any program

  1. Will a provider review my labs before prescribing?
  2. Will anyone measure whether I am losing fat or muscle?
  3. If I have side effects, who do I call?
  4. Will the same provider manage my dose over time?
  5. Do they offer FDA-approved brand-name medication, and explain the differences if a compounded option is considered?

At Peak Performance

Eddie reviews your labs, and we scan your body composition at your regular visits.

You call or message our office when something feels off, and the same provider manages your dose.

Eddie can prescribe FDA-approved brand-name options and walks you through the differences if a compounded medication is considered.

You get a real exam room in Johnson City and a provider who knows your history.

Life after

What life looks like on the other side

The first change most patients describe is quiet. Food stops running the day.

For a lot of patients, the goal is not just a number on the scale. It may be an easier walk at Warriors' Path, keeping up with the kids at Winged Deer Park, feeling more comfortable through an ETSU game, or improving health markers you are already tracking with your primary care provider.

That is what we plan for, starting at the first visit.

Questions

Frequently asked questions

How much does medical weight loss cost in the Tri-Cities?

At Peak Performance in Johnson City, most patients pay $300 to $500 per month depending on medication and dose. That includes the evaluation, labs, medication, follow-ups, and body composition monitoring. There are no contracts or cancellation fees.

Why does it cost more than the online GLP-1 programs I see advertised?

Many online programs advertise a low monthly price. Before you compare, check what that price covers. Are labs included? Does a provider review them with you? Will anyone measure whether you are losing fat or muscle? Can you reach someone when side effects hit?

At Peak Performance, most patients pay $300 to $500 a month. That includes your evaluation, labs, medication, follow-up visits, nutrition guidance, and body composition scans, with no contracts or cancellation fees.

Where can I get a GLP-1 for weight loss in the Tri-Cities?

Peak Performance prescribes GLP-1 medications, including semaglutide and tirzepatide, at our Johnson City clinic after an evaluation and lab work. Eddie Hughes, FNP-BC, manages your dose over time. Patients come from Kingsport, Bristol, Elizabethton, Greeneville, and across the region.

I am already on a GLP-1 from another provider or an online program. Can I switch to you?

Yes. Bring what you are taking, your current dose, and how it has been going. Eddie reviews your history and labs and decides whether to continue your current dose, adjust it, or change medications.

People often switch when insurance drops their coverage, when progress stalls, or when they want a provider they can actually see.

Who qualifies for medical weight loss?

Anyone who wants to lose weight can start with a consultation, lab work, and a body composition scan. Whether medication is part of your plan depends on your numbers and your health history.

Wegovy and Zepbound are FDA approved for adults with a BMI of 30 or higher, or 27 or higher with a weight-related condition like high blood pressure, type 2 diabetes, or sleep apnea. GLP-1 medications are not used if you or a family member has had medullary thyroid cancer, if you have MEN 2, or during pregnancy.

Should I tell my regular doctor I am on a GLP-1?

Yes. Keep your primary care provider for your other health needs, and let them know what you are taking. As you lose weight, what you need from other medications can change, especially diabetes and blood pressure medications. GLP-1s also slow digestion, which can affect how some pills absorb and matters before any surgery or anesthesia.

Eddie reviews everything you take before prescribing, and your weight loss care works alongside your regular doctor.

Which weight loss medication is the most effective?

In a 72-week head-to-head clinical trial of adults with obesity who did not have type 2 diabetes, tirzepatide produced an average weight reduction of about 20 percent, compared with about 14 percent for semaglutide, at the maximum tolerated doses used in the study (SURMOUNT-5, PubMed). That does not mean tirzepatide is the right medication for everyone. Your health history, side effects, coverage, cost, and response to treatment all matter when choosing an option.

What kind of doctor will prescribe weight loss medication?

Physicians and board-certified nurse practitioners can both prescribe weight loss medication in Tennessee. At Peak Performance, Eddie Hughes, FNP-BC, a board-certified family nurse practitioner, evaluates you, orders labs, and manages your prescription.

Do I still have to diet on semaglutide or tirzepatide?

You will eat less because you are less hungry, but what you eat still matters. In the clinical trials, medication was paired with a reduced-calorie diet and more physical activity.

Eating enough protein and strength training also help protect your muscle. Eddie gives you protein and meal guidance built around your numbers.

What are the side effects, and how do you help with them?

The most common side effects are nausea, diarrhea, constipation, and vomiting, mostly while your dose is going up. Doses start low and increase gradually to keep these manageable.

If they show up anyway, call or message us. We have specific ways to handle them and can slow your dose increases. There is no reason to push through it alone.

What does the first month feel like?

Most people notice their appetite drop and the food noise get quieter within the first few weeks. Some also have mild nausea, constipation, or fatigue while the dose settles in.

Eating smaller, slower meals, getting protein first, and drinking plenty of water help. If anything feels off, call or message us, and we can adjust your plan.

Will I lose muscle on a GLP-1?

Some of the weight lost on any diet is muscle. Eating enough protein and strength training 2 to 3 times a week both help you keep it.

We scan your body composition at your visits, so you can see whether you are losing fat or muscle, and we can adjust your plan before it becomes a problem.

What if my weight loss stalls?

Plateaus are normal, and they are one of the main reasons to have a provider. Eddie looks at your dose, your eating, your activity, and your body composition scan, and changes the plan.

You work through it with us instead of guessing on your own.

What happens when I stop taking GLP-1 medication?

Most people regain weight if they stop without a plan. Studies that followed people after they stopped semaglutide or tirzepatide found most regained a large share of what they had lost within a year.

That is why we plan maintenance from the start and keep checking in while you come off or move to a lower dose.

Do I have to stay on medication forever?

Not always. Some patients hold a lower maintenance dose long term, the way you would manage blood pressure. Others taper off after they have built the habits that keep the weight off.

Eddie will be honest with you about which fits your situation, and you make the decision together.

How long does it take to lose 20 pounds on semaglutide?

There is no reliable 20-pound timeline. Weight loss depends on your starting weight, dose, response to treatment, nutrition, activity, and side effects. Semaglutide starts at a low dose and increases gradually, so weight loss usually builds over months instead of following a steady weekly rate. Eddie tracks your weight and body composition and adjusts your plan to how you respond.

Does insurance cover weight loss medication?

Sometimes. Coverage depends on your plan and often on your BMI and conditions like type 2 diabetes, high blood pressure, or sleep apnea.

If your insurance covers a brand-name option, Eddie can prescribe Wegovy or Zepbound. If it does not, he will walk you through your other options and the monthly cost.

Are there weight loss clinics in Kingsport, TN?

Yes, Kingsport has several. Many Kingsport patients also drive about 30 minutes to Peak Performance in Johnson City for provider-led care with labs and body composition tracking. Our Kingsport weight loss page covers what that looks like.

Do you see patients from Virginia?

Yes. Patients from Bristol, Virginia, and Abingdon are seen at our Johnson City, Tennessee clinic.

Do I have to come in every week?

No. Most patients give themselves their weekly injection at home. You come in for follow-ups, dose adjustments, and scans on a schedule Eddie sets with you.
